Create custom academic history page

Detailed description of desired deliverable:

Updating Academic History Section from Slate’s Original Page to a Custom Widget (similar to Cummings, see below).

Also add question about GPA (make it required to either put in their GPA or check-off a box that says “Institution does not use traditional Grade Point Average (GPA) system.”  

GPA on a scale of(e.g., 4.0, 4.3, 5.0, 15, 100); Do not recalculate GPA, and do not report if not printed on transcript

At some previous point, we had set up the ‘primary’ fields…and we set up a bunch and clearly crashed work that had already been done:

Luckily, nothing has any data in it, so it is just a matter of deleting any that are not being used.

Open questions: does anyone care about ‘primary grad school’?

*it is challenging to build school-based widget forms with the right logic to only show on the correct application. I think separate widgets are the way to go, embedded on the same master custom academic history page.
