Desired Deliverable
A single form in each instance that is available to incoming students to make updates should they choose to do so. At this instant, there is nothing automated about changes students ask for; @Elizabeth Storrs is the ‘admin’ on the forms, so if/when forms are submitted, she will be working with the appropriate registrar to make any changes.
Ultimately this will flow without human intervention.

11/26: The final version of the form has been set up in test--we haven't really talked about how you are planning on letting students know about it, so as a demo I added it to the checklist of enrolling students (as optional), showed it in the 'forms' section on the student portal, and restricted access to it to the students who have it on their checklist. . There are 5 students in test who have been admitted, so they all have the form: Kait Bell, Reto Giezendanner, Damla Hauser, Nahin Mubassira, and Malvika Pradhan.
If you like that, I am happy to set it up for you in production. Other options include calling it out more obviously in a text block in the portal, or pivoting away from including it on the checklist at all.
